2026-2027 Pre-Participation Medical Requirements
Welcome to all new student-athletes, and welcome back to our returning student-athletes.
We are beginning the process for the 2026–2027 pre-participation medical clearance. Please read all information carefully, as requirements are updated annually.
Important Instructions
- Select the correct participation year, including all previous years on the team.
- If you took a year off, you must follow the 3rd-year requirements.
- Requirements must be completed by the following deadlines:
- June 30th for 2nd, 4th, and 5th-year student-athletes
- July 30th for 1st-year, 3rd-year, and transfer student-athletes
Failure to meet these deadlines will delay your medical clearance to practice or compete at Montclair State University.
ATS Medical Profile
Use your
ATS (Athletic Training System) profile to enter demographic information and upload all required documents.
Our staff manually reviews and verifies paperwork Monday through Thursday. Once approved, your status will be updated under the “Paperwork” tab, and you will see
“Cleared To Play” if your medical profile has been reviewed and accepted.
Do not contact the Sports Medicine staff to verify your profile status. With over 600 student-athletes, it is not feasible to respond to individual inquiries. Please allow up to one week for processing.
Concussion Testing
A member of the Sports Medicine staff will contact all first-year/ transfer student-athletes regarding concussion baseline testing requirements.

All forms must be uploaded to your ATS profile.
1st-Year / Freshman Student-Athletes
Note: Sports Medicine forms are separate from those required by the University Health Center. Do
not upload Health Center forms to ATS.
